Dog grooming includes, brushing, bathing, clipping if necessary, ear cleaning/plucking and nail trimming. All pets are hand dried, not in a cage. There will be an additional charge for difficult pets, severe matting and/or flea/tick removal.
For the safety of other animals in the shop as well as your own we require that your pet have the following vaccinations:
Rabies, DA2PP ( canine distemper/parvo), Bordetella (kennel cough) within the last 6 months. These must be made available upon request before or at the time of your appointment. With your signature on your information card you are stating that your pet is current on these required vaccinations and may be held liable if your statement is not true and correct.
Health/Medical Problems or Senior Dogs Grooming procedures can sometimes be stressful especially for a senior dog or dogs with health problems. Because of this, grooming can expose hidden medical problems as well as aggravate a current one, whether it be during or after the groom. Because senior dogs and dogs with health problems have a greater risk of injury, these dogs will be groomed for cleanliness and comfort as Furr-Nley’s Pet Spa will not compromise any dog's health and/or wellbeing to achieve “the perfect groom”. The dog owner will advise Furr-Nley’s Pet Spa of any medical, physical, emotional issues, allergies, sensitivities, or pre-existing conditions. These conditions may include, but are not limited to prior surgeries, hip and/or joint issues, warts, moles, ear infections or skin problems. The owner will also inform Furr-Nley’s Pet Spa of new conditions as they arise. Furr-Nley’s Pet Spa will not be responsible for accident or injury to an elderly or health-compromised pet during their grooming. By signing below, the owner is stating that Furr-Nley’s Pet Spa has permission to groom their senior dog and has discussed any and all (past and present) health problems.
We do groom dogs with fleas and ticks but we need to be told ahead of time to make sure we don't expose any infected pets to others that may be in the shop. Failure to inform us will result in additional charges which include but not limited to treatment of your dog or areas that may have been exposed. If fleas or ticks are found on your pet during the grooming process your pet will be treated with a Flea and Tick treatment to kill the parasites and you will be charged an additional fee. Additional charges will be applied to your final bill. Ticks found will be removed for an additional charge. If tick are found we strongly advise your to take your pet to be checked by your Vet and have your pet tested for Lyme Disease. Please not that parasites are a health hazard to your pets as well as humans.
If your pet is matted, all mats will be removed. If brushing out is possible then a charge of $60.00 per hour will be added to the base groom price. If the pet needs to be shaved, due to matting, extra charges will be added to the base price. Severe matting or brushing of mats may cause areas of dermatitis, irritation, brush burn, nicks and cuts, and can be tedious, painful and stressful for your pet. Furr-Nley's Pet Spa cannot be responsible for any injury during the process. If Furr-Nley's Pet Spa determines that the injuries should be medically treated owner will be notified by the groomer and all expenses will be charged to the client. I acknowledge that my pet may need to be shaved today. The remaining length will be determined by the tightness of the mats.
Shaving double coated breeds is not recommended at Furr-Nley's Pet Spa. Shaving these breeds can cause the coat to not regrow as well as changes in color and texture. Shaving down does not relieve your pet from the heat. It can make them less able to regulate their body temperature. If shedding is an issue, other methods are recommended.
There is always a possibility of an accident. Grooming equipment is sharp. All efforts will be made to use care and caution during the grooming process. Possible problems could occur including cuts, nicks, scratches, and quickening of the nails. In most cases this can happen when a pet is wriggling or moves around. Every effort will be made to insure your pet is groomed as safely as possible however, some pets may have reactions to grooming products or procedures such as shaving or ear plucking.
I understand that if any injury occurs during the grooming process which requires veterinary care I will be responsible for these charges. I understand that my pet will be tethered using a non-tightening loop during the process. Tethering with non-tightening loops is safe and help prevent your pet from slipping or falling.
Please give 24 hours advance when cancelling an appointment. Cancellations made less than 24 hours advance notice may be subject to a 50% re-booking fee. No call/No Show for appointments may not be allowed to re-book for an appointment.
Please make every effort to be on time for your appointment. Pets arriving more than 15 minutes past appointment time, unless prior arrangements have been made, may have to re-schedule.
An approximate check out time will be given upon arrival, however we will call you when your pet is finished. If you come back before the groom is finished and the groomer feels she has lost control of your pet, you will be charged full price and your pet will go home unfinished. If an early check out time is needed prior approval must be made with the groomer. Any pets left longer that 2 hours after the finished call has been made will be assessed a day care fee of $25.00.
I understand that any images of my pet(s) in a media format (pictures, video, web cam, etc.) are property of Furr-Nley's Pet Spa and may be used for publicity and advertising, etc.
Furr-Nley's Pet Spa has the right to refuse care for a pet that shows aggression toward staff or other pets, displays high anxiety behaviors such as destroying property, excessive barking, and or excessive urination, or defecating in crate. Please make sure your pet has been taken to relieve themselves before grooming. We reserve the right to stop services for such pets at any time before or during the grooming process. You will be charged the full price for the grooming.
Your satisfaction is important and if your are unhappy for any reason please let us know at the time of pick up and allow us to try to remedy the problem. Once your take your pet home however, we only accept return visits for 24 hours after your appointment. Any adjustment after that time will be charged a grooming fee. We strive to provide excellent animal care at Furr-Nley's Pet Spa. We want your pet's experience to be as un-stressful as possible. We appreciate your understand with our policies and procedures. This helps insure that all pets and clients receive the highest lever of care your deserve.
Please read carefully. This agreement includes a Release of Liability and Waiver of Legal Rights and deprives you of the right to sue Furr-Nley's Pet Spa, LLC or any related parties within the establishment.
The person who brings the pet into Furr-Nley's Pet Spa shall be the authorizing agent for all decisions relating to the care of the pet. This person shall also be the witness to the particular cut, any matting, and the current length of the hair on the pet.
I hereby acknowledge and agree that all the information provided in this grooming and consent for is complete and accurate to the best of my knowledge. I further acknowledge and agree I have read, understand and agree to all terms and conditions contained the grooming policies, procedures and release, Waiver of Liability, Assumption of Risk and indemnification Agreement (the Agreement) as they may be amended from time to time, which are fully incorporated into this application by reference. I hereby execute the agreement for my pet, myself, and my heirs successors and representative and assigns. I further attest that if I am not the sole owner or representative of the pet subject to this agreement that my signature is sufficient to enter in this agreement for and on behalf of any other owner or representative.
This agreement covers the duration from the date of signature to date on the grooming/client information card.
